  • Date of birth
  • Contracted hours
  • Qualifications and mandatory checks (with expiry dates)
  • Visit history
  • Normal work availability (including rotating shift patterns)
  • Planned visits
  • Client exclusions
  • Attachment of documents (including CVs and photographs)
